You are eligible to register for a Preventing Diabetes account with access to your personalised health record if:
- You are over 18 years old.
- You are registered with a GP practice in North West London (Brent, Harrow, Hillingdon, Hounslow, Hammersmith & Fulham, Westminster, Kensington & Chelsea and Ealing).
- You have been diagnosed with Non-Diabetic Hyperglycaemia (high risk of type 2).
However, anyone registered with a GP practice in North West London can create an account and access Preventing Diabetes resources and eLearning modules. Therefore if you are a carer or a healthcare professional living in North West London and would like to log in to view our online education courses, please feel free to do so.
